Kimberly Diaz, RN, is a nurse, dementia care expert, Certified Dementia Practitioner and trainer, Certified Placement Specialist, case manager, Certified Death Doula, and Certified Elite Brain Health Clinician. With nearly three decades of nursing experience, she is the founder of Care Matters, LLC, Care Matters Registry, and Caring Minds Dementia Consulting & Training. Kimberly helps families navigate dementia, aging, care at home, senior living and memory care placement, healthcare coordination, advocacy, and end-of-life decisions. She is also the author of “Where Do We Go From Here? The Complete Dementia Care Guide for Family Caregivers.”
She founded Caring Minds Dementia Consulting & Training to equip families and professionals with the guidance, tools, and insight they need throughout the dementia care journey.
